You are being invited to participate in a survey of physical therapists who perform fall risk assessment. This survey seeks to identify current practices and considerations during fall risk assessments, including factors related to cognitive impairment and hearing impairment. The survey also explores the knowledge and practices of physical therapists related to cognitive screening, hearing screening, and in working with patients with cognitive impairment and/or with hearing impairments.
This survey is carried out by an undergraduate student at the University of Colorado Boulder as part of an honors thesis.
The survey should take about 10-15 minutes to complete. No personally identifiable information will be collected from you. Participation in this survey is completely voluntary, and the survey can be exited at any time. By beginning the survey, you consent to participation.
